WCHC believes in empowering patients to take control of their health.  Patients can take advantage of many educational and self-management programs located in our service area.  WCHC has conveniently attached a list of some of the most popular self-management classes being offered in the community and links to other community resources.

Chronic Disease Self-Management Program: An evidence based, 6-week program that meets once a week for two hours.
