Sliding Fee Program
As a Federally Qualified Health Center, WCHC offers a Sliding Fee Program for all services based on income and family size. This program can significantly reduce the amount you pay for healthcare services. We believe your care should be high quality, accessible AND affordable.
All WCHC patients are eligible to apply for the Sliding Fee Discount (“SFD”). Even patients that have insurance may apply for the SFD to help lower the cost of non-covered services.
We encourage you to review the eligibility requirements below and apply if you qualify, or ask for help determining your eligibility at any of our locations. Applications are also available on site, and we have staff available to help you complete your application. You may also apply online at the links below.
Proof of all household income is required, and we strongly encourage applicants to provide a copy of their most recently filed Federal Tax Return. Other documents to be used are listed below:
- W-2 forms
- Income tax returns
- Social Security notice
- Current pay stubs
- Unemployment award notice
