West Cecil Health Center is proud to announce we have adopted a team-based model of care. Team-based care emphasizes care coordination and communication, improves the quality of care, helps to lower medical costs and achieve an excellent patient care experience. As part of our commitment to provide you with the highest quality care, we partner with you and collaborate with your other providers to tailor care specific to your needs. This model of care is often referred to as a Patient-Centered Medical Home. The foundation of a home is the relationship between the patient and provider based on respect, trust, confidence, and care.
What can you expect from us?
We are committed to providing a safe and healthy healthcare environment. We will partner with you to make decisions about your care and utilize evidence-based medicine to guide our treatment options. We will collaborate with you, your authorized representatives, and other healthcare providers to create an individualized plan of care tailored to meet your needs. We understand the importance of the ongoing patient-clinician relationship and we will do our best to provide you options for care and to support your decision to see a particular provider or care team.
What is your role as a patient?
We encourage you to communicate openly with your healthcare team. Ask questions and speak up if you have concerns. We are accessible to you via phone, mail, fax, or online via the portal. Keep us current on changes in your medical conditions, treatments, medications, or specialists. Partner with us to set goals that will help you achieve better health.
